The timing of events and communications with your boss will be important. I would make your own accurate notes as to how you got into this situation, but do not share them on any public forum!

I doubt a letter from work will help. The only sort of letter that will work is one where they accept liability for the abuse of your pre-printed signature. I don't think you'll ever get such a letter because it drops them in the ----.

I would write your boss a letter 'reminding' him that your signature cannot be used until you have inspected the work, and ask that all pre-printed forms be withdrawn from service.

when i tested for a boss of mine , i had 2391 etc but was only apprentice, i never signed the forms, and i could see when i handed him them, he noticed but said nothing. but he put his results into a package from niceic and it confirmed results even tho i could tell by looking at them if something was obviously wrong... the whole lot a load of poo. the only downside i see is what way someone installed something... and you wernt there and therefor the boss still used your name..
 
Pre-printed signatures on test certificates should never be allowed in the first place, these are official documents. Doing so, opens the doors to all sorts of miss-use and forgeries. Signatures should always be hand written in blue ink, (or perhaps a company preferred colour).

maybe the feds tracked him down :)

I think he has realised his company has put him in a potentially very sticky situation!!
Unfortunately you can't trust anyone these day's, least of all your own employer, if they are out to make a buck.
 
With regards to the pre signed documents if it is a photocopy of the signature then its not legal my understanding is it must be made by a pen
