Boiler isolator, under boiler on above cupboard

On a boiler that is boxed in (kitchen units), should the isolator be below (i.e tiled around), or can it be above the boiler on the kitchen unit. Both would be accessable, although not as convenient!

Should the isolator be switched, or fused only (i.e remove fuse to isolate)

As long as it is reasonably accessible and obvious that it controls the boiler, put it in the most convenient place. It must be a switched DP FCU with a 3amp fuse!
