where there are more than one reference method ie. one section run in an insulated wall and alsoanother section in plaster on a masonary wall.

As reference methods are used along with other factors, to calculate the (Iz) of a cable, to err on the side of safety I would take the factor with the most detrimental effect on the cable.

Table 4D5 is for T&E cables above ceilings etc.
 
Yes - you should use the most 'severe' section of your cable run when it passes through different environments.

I think the question might be which is the correct reference method when the cables are run between floor and ceiling where there is NO insulation - in which case is the answer reference method C - clipped direct, or number 40, single core or multicore cable in a building void, leading to reference method B
