I've seen many versiions of certs with different layouts, which is the correct one or doesn't it matter as long as it has the required fields.

The certs that the scheme providers use, are based on the certs in BS7671.

They then redesign and add extras to suit them.
 
It really doesn't matter as long as you have what is on the model form in the IEE Regs and as Jason said some put other tests on as well.

If you look at them they will have on them somewhere in small print that they are based on the forms in BS7671:2008 or just BS7671

Would the NIC for example accept the forms that are on the IEE website for instance, or does it have to be their own forms.
