anyone got a blank form for a certificate for the installation of data (cat 5) cables - as a favour we installed some when on a shop redevelopment, and now the head contractor wants a certificate for the data cables. We tested them all through with our ethernet testing gear, but have no forms to record the data on.

Assuming you have recorded the results of those tests then just make up a table in excell with each point listed against it's test results. Then make a title page to go in front of it with whatever other relevant information you need.

When you say you tested with your Ethernet testing gear are you talking a Wirescope or fluke type tester or a Ethernet pair flashing light tester? If you have used the correct tester you can use linkware free software. If you have used the flashing pair light tester then you will not have the Pair Data, Insertion loss, Next, PS Next, ACR-F, PS ACR-F or the RL readings that the contractor requires and will have get someone to certify for you.
