Would the lack of an RCD go in here although when installed it wasn't a requirement?

If your work required rcd protection then you should provide it because regardless of when the existing work was installed, you install to BS7671:2008.

In amendment 1 there is provision for omitting an rcd on minor works subject to risk assessment but you'll have to wait a good while for that.
 
Like for like would not require upgrading....in which case a lack of rcd protection could be noted in the 'comments on existing' box....a departure would only be the case if the work you carried out did not comply.
