Bit of a boring subject, and this form is a pain in the arse. But does anyone know who has filled one out before, that in section 10 " Employer or sponsor endorsement and recommendation ", does this section have to be filled out if you are a self employed subby?? If it is, can it be filled out by another spark, that you work with??

Yes it has to be filled out, and yes you can get it filled out if you are a subby. Get whoever you sub to, ie a contracts manager to fill it out and don't forget to get them to sign the rear of your photo too.
