Looking for the above, does anyone know of a supplier please?

I don't think you can get them TBH. Might be easier to just sink in a twin (i.e. 2 gang) back box and use an FCU and a single socket. Would have to enlarge the hole slightly as those boxes are a bit wider than a double back box. And make sure you're not creating a 'spur off a spur' situation.
 
Crikey, I've never seen one of those. I've got an FCU and a socket of that make but not a combined one. Funny that they didn't make the switch dollies match - the FCU is D/P but the socket is S/P - they just used the type already designed into the separate items. The 4-lug fixing dates it!

The rcd spur being fitted is probably because there wasn't rcd protection of that supply.
