What's the norm ? have a site with 5 off 3 phase distribution boards some 6 and 8 and a couple of 12 ways.Every one is labelled differently.I have just put a new Wylex board in and it starts bottom left column as number 1,L3 1,L2 1,L1 then right bottom number 2,L3 2,L2 2,L1 then going up number 3 is on the left column,4 on the right.
All other boards are just numbered all over the place,top to bottom and bottom to top in single number .Boards have 3 phase breakers labelled ie 22,23,24 for another board or machine but this should be a one number and L3,L2,L1 or R,Y,B.

What's best practice ? is there a reg ? so many circuits are unknown it would make sense to start fresh and get it right.

Time to play on excel me thinks.

I number like this:

1/L1 3/L1
1/L2 3/L2
1/L3 3/L3
2/L1 4/L1
2/L2 4/L2
2/L3 4/L3

If on a periodic inspection, I change all numbering to that format.
 
I label each one the same as above you would read a book like that so why complicate things? I haven't done a lot of 3 phase work. But what i have notice is some people will label their conductors in number sequence so cables mark 6 on line, neutral and earth are for circuit 3L3
