I would like to help you but don't really understand what you need this information for..What country are you located in? The information that you requested will not be the same every time,it will depend on where you are,the transformer type,supply system grounding etc. The installation is carried out by your local electricity supply company and sometimes a little information can be more dangerous in the wrong hands.......so please leave to the professionals.

Agree as the is no such thing in the UK as medium Voltage it's either extra-low voltage, low voltage then high voltage.
