This any use to anyone? Knocked it up last night/this morning. You can fill in the form using Adobe Reader, or other popular PDF viewers. If you use Adobe Reader 11 (or presumably later) and some other viewers, then you can save the information you type in on the form as well as print it (earlier versions of Adobe Reader, you could just print it).

I have to admit, I am not sure of the copyright implications of this. I have typed in everything on the form (I've not used any cut and paste), I have tried to recreate the model form in the BYB as accurately as possible (with the addition of "Form no" at the top, which I thought would be useful for a reference number - it was there in the amendment 1 model forms). Maybe I should ask the IET. My understanding is that they actively want people to use the model forms, and they release their own - they are just a bit behind the curve on this (no surprise there, then).
Hence only in the trainee section at the moment.
All done using open source software.
