minor electrical installation works certificate template?

Hello all

I'm trying to find a template of a minor electrical installation works certificate that comes in a word format or something I can put information into onsite using my laptop. I have just registered with Elecsa and imagine they have something on their website but they haven't finished setting up my account as yet.
