would just like to know how to start each write up
i dont want to make it to long you no
so do i just start write up up with job description/method statement/risk assesment/picture of bits of job/isolation ETC

You need to speak to your Assessor. He or she should give you clear guidance on what you need to do to gather evidence whether it is written, photographic, witness statements, and so on.
