Hi can anybody help me with a generic variation order?

My works don't have variation orders and I need a variation order as part of the minimum evidence required

To be honest I think this is mostly done via email these days.

Details would be:

Who has instructed the variation.
Time and date it was instructed.
Detail of what the variation is.

Yes they mostly are, or verbal agreement.
