PAT Test Sheets

Guys probably a stupid question but where can I get some blank PAT test sheets free.
I need to PAT test a fridge freezer and electric heater in my sisters house for her to rent out.
The estate agents insist she had the 2 items tested and conveniently agreed to do it for her for £80 so I said ill do it just need some sheets for hand written copies cheers guys.

Look in the PAT testing regs book, at the back there is a standard test sheet. Just get on excel and type one up.
