PV DE-Commissioning

I've been asked to De-commission a legacy in roof system. 8-9 years old small poly tiles. Roof is leaking and they've decided to re-roof the house without the old system.

Customer has been told we need to provide a decommissioning form. Anyone heard of this before and where might i get one?

In the past I've used the form out of the G59/3 document, which was sent to me for the site in question by Scottish Power. It was section 13.4 named Generating Plant Decommissioning Form. Not sure if this will be what they are after...

Customer has received one from eon. For future ref its called a:

Notification to Ofgem of FIT Installation Decommissioning (version 3).

I tried to upload it here but failed.
 
Ah, for the billing side of things that makes sense. I'll remember that,thanks. In my case we were replacing a system so it was a bit different and only DNO needed to know of the change - supplier seemed to work it out on their own, perhaps as it was not on fit/ROC scheme previously...
 
As above, you also need to notify the DNO of de-commissioning, the ofgem / fit form is only part of it, and if it is a legacy system, probably doesn't receive any FiT anyway...
