quick periodic question

i was just looking over some notes because ive got an exam today and was wondering if someone could confirm if this is correct.

if im doing a periodic on an installation with 2 dbs the total number of certs would be 4 comprising of:

periodic inspection report
schedule of inspections
schedule of test results (db1)
schedule of test results (db2)
