Reconnection of Supply??

when the local suppliers call to reconnect an existing supply(meter plus i think) what paperwork will they need to see,i seem to recall a yellow form asking for max demand etc?? but im not sure..help

Best thing to do is give them a call and ask, as they all operate differently.
