One of my customers has about 24 downlighters in their kitchen/diner/utility. They all fitted in "large" dish type holders and some of these appear to be glued in.

As you'll see from the photo they aren't that nice to look at.

My question is, is there any other option, other than ripping the ceiling down, to replace the fittings as they are about 20 years old and they are getting increasingly unreliable.

Yeah there is but it takes quite a bit of time and effort. Cut two pieces of baton and put them in the ceiling as close to the edge of the hole as possible, screw through the plasterboard into the wood so its held. Cut a piece of plasterboard the size of the hole and screw through the piece you have cut out into the bits of batton so it is held inplace. Put bonding and finefinish it and then after a few hours you can cut your new holes inplace. It takes quite a bit of time but looks good
 
screw fix used do the adapotors too, has a quick look and cant see the plate on its own but they do have a load of fire rated complete lights that should do the trick - in various colours - put 'converter plate' in their search box - pointless putting alink as this site wont display it
 
Like the others said, not as good as a new ceiling, but these downlight converter kits are also available from TLC, part no ML HCW for the white ones. Very cheap, but I've used them and don't think they look too bad.
 
I fitted some of these plates last week, one problem I found was the LVD I used were fire rated, and too heavy for the springs supplied with the plates. The standard springs worked, but as they had to grip at the end of their reach, didn't hold the plates and lights in place very effectively... I got around this by fitting a couple of batons in each hole first. Ball ache with 12 to do, but worked.
