I have stiil seen recently sparks from an niceic approved firm putting tape over breakers,it is jut not good enough.I wont trust anyone but myself and if i dont have my lock off kit to hand i will remove all live conductors from inside the enclosure.Never trust anyone is what they should put in bold letters on there handouts.
 
I got a tingle couple of years back,working on a lighting circuit,breaker off,didnt have lock off kit with me,someone walked in and i didnt see them switch it on.what the hell!!!I told myself off for that mishap,and it will never happen again of that i am sure.
 
Had a serious senior moment the other day which this has OP brought back (well flashback).
Basically many machines moving i checked phase rotation, isolated, pulled cables and breakers out of the board, then reconnected new machines with appropriate breakers, basically lots of different sized machines moving around the factory.
Anyway - managed to create a new type of combi cutter when i cut through two phases of a live cable on a C20 breaker, what a bang......
could not understand what went wrong, i had been so methodical and made no mistake about pulling cables and breakers out of the board.
It turned out one machine had been moved a couple of years ago, whoever did it just taped over the wires but was still powered up.
client offered me his pen tester which was a nice touch but bloody hell, i was 3m up on a scissor at the time and convinced myself it was a cable i had previously pulled out of the board.
Anyway - plonker of the week for me, not something i am likely to repeat anytime soon especially as its cost me my best cutters
 
Got to be said that you should not trust any one else got caught out as we talking over raido link board some distance from work area and just spotted in time that it had been swithched back on thank god still we live and learn we hope
