For £75+VAT (probably the cheapest form software) just buy cet software. Not only does it allow you to quickly fill in the forms, it also double checks against the permissable values for the selections made. The best bit is you have no fear of overwriting a previous file if you forget to "save as" as its all too easy to loose your certs that way.

There are many about, but I like the ease and functionality of PIR Form
