Is it all the wires in the correct connectors or do you have to leave the live wire out and link the neutral to the live like it is in the three phase adaptor.
 
If you want to test continuity and polarity of 110V cables you'll need all three poles connected separately. The problem is that you then get a naughty adaptor that allows you to plug 110V devices into 230V. You can reduce that risk by making it a silly colour, plastering it with labels and tying it to the PAT tester. If it is only for testing earth continuity and insulation with L&N linked in the tester, you could just use one pole of the plug to feed both live poles of the 110V socket.
