Hi, Im doing my DI assessment next week and need help on areas of filling out the forms?

I`m filling in an EIC for my smaller of the 2 jobs. I`ve added a light circuit for an outside light.

Q1. On page 1 of the EIC do I just write the clients name where it says "DETAILS OF THE CLIENT"?

Q2. Do I put "domestic premises" in the "description of installation" part?

Q3. A) what to write in the "EXTENT OF INSTALLATION COVERED BY THIS CERTIFICATE" for the new circuit?
B) Same question for my major job which is a CU change?

Q4. Any examples of " DETAILS OF DEPARTURES FROM BS 7671" for both jobs?

Q5. In the "MAXIMUM DEMAND" box, would it just be the current for the light I fitted on the new circuit?

This is something that you should already know if you are doing the assessment.
Have a look at the OSG as I think it has completed examples.
1 clients name and address
2 yeah
3a new lighting circuit no.X, make sure it is described clearly,
3b CU change only
4 none as it should be installed to BS7671 can't really self notify otherwise as the self notification is for jobs to BS7671.
5 whole installation I normally put not known but better to get an idea of what may be in use and estimate an appropriate value.

I don't know which forms you are using, so can't be specific.
Generally, the client will live at the address where the installation is being inspected.
However especially with many installations being in rented properties, it may be that the client lives elswhere.
As such, you could just put the clients name, or their name and address, it's up to you.
As for the description, I would consider putting something more descriptive, such as first floor flat, bungalow or perhaps lock up shop or cafe.
You really need to be quite specific when detailing the extent of the work, something along the lines of: 'One 30mA RCD protected radial circuit supplying six BS1363 double socket-outlets'.
With a CU you would want to state whether you are replacing an existing item, whether you are providing RCD protection for any of the circuits, upgrading erthing and bonding, any new circuits etc.
Departures are instances where it is decided at the design stage not to comply with the Regulations. Departures must offer the same degree of safety as would be provided by compliance with the Regulations. An example of a Departure would be for instance when replacing a CU in an installation where there is no CPC for lighting circuits. You might decide to protect such circuits with a 30mA RCD, ensure that all fittings and accessories are plastic, and place a lable at the CU warning of the lack of CPC and advising that class I fittings and accessories should not be installed.
In the instance you describe, it would be the maximum that the fitting is rated for. If for instance you were to install a 60W incandescent lamp, the maximum demand could be 100W, so you would use that when determining the maximum demand.
 
On an EIC the Maximum Demand is the designer's estimation of the maximum load demand of the whole installation expressed in kVA or A/phase and takes in to account diversity.
