Hi all, first time that i have been called out to a potential fire in a consumer unit.
The Line tail was loose at the main switch, resulting in serious scorching around this area and welded a few circuit cables together. Service fuse had also ruptured.

The owner has spoken to his insurance company and they have requested a report including photographic evidence of the damage.

Anyone submitted such a report before, any pointers/tips?

Everything has been fine since he moved in approximately 12 years ago as far as he is aware.

You write an account of the condition of the board as found, include some photos. Describe the damage sustained and what is required to rectify. You could include that further tests would need to be carried out in the remainder of the wiring installation to ascertain its integrity for continued use. It maybe that they require you to do an EICR of the installation along with a report on the damaged cu.
pergaps speak to the insurance company direct to confirm the details they wish for.

I suppose the label advising when the next PIR / EICR is due was burned-off in the fire .......

I was very unsure about this the first time I had to do a similar thing, the idea of a written report conjures up images of endless pages of text etc. I do call-out type work for a letting agent and will normally do such a report for each job, I always follow this basic layout:

I attended site on (date)

The (owner/tenant) had reported (symptoms)

On initial inspection I found (a mess)

I carried out testing/investigations and found (a big mess)

This is likely to have been caused by (a 5 week wonder)

In my opinion this (was immediately dangerous/requires improvement)

I carried out (work to rectify/temp repair) and supplied (materials)

This requires (permanent repair if repair was temporary) provide quote if appropriate

I recommend (further works/EICR)

The cost incurred so far is (£millions) made up as follows: (list items to be charged)

Include pictures, label them if necessary

Obviously for some jobs it is a simple 'tenant reported light not working, I found a blown lamp and replaced it' and other times it is somewhat more detailed.

Reports should be kept clear and concise, a long wordy report will likely not get read properly and likely be of no use to anyone without a good knowledge of technical language

Just be factual I do repairs in rentals and a digital camera is a good friend so its

1. Called out to property by xxxxxx
2. Tenant / customer /landlord reported that xxxx happened
3. Investigated / checked xxxx and found the following
4. Suspect problem was xxxx being loose causing xxxx damage please see pic 1
5. Also checked and made good further loose connections on xxxxx see pic 3
6 Repaired xxxxx and replaced xxxx see pic 3

And so on no need to embelish it and remember you don't say this was the problem you suspect this was the problem why cos some clown will come in and try and shoot you down also added some samples that I found
