On an installation there are two main switch’s one in a 12 way DB, another is an exel switch fuse supplying two DB’s. What would you put down as the main switch on the cert on page 2?

i'd be doing a schedule of test results for each DB. then on each put the main switch details for each. on the inspection section, note saying "see DB schedules"
 
my bad. was looking at an old schedule where each had a main switch details for the DB.
 
If it has no isolator from DNO and the origin of supply is split then the most comprehensive way would be to complete 2 seperate certs.

It's no different to having a unrestricted and off peak supply at a house, some people do it on two certs and others just add a schedule of inspections and a schedule of tests, not right really but not a big deal in my eyes.
