C2 for the colour alone.

DEXION for mounting probably warrants a C1, ok for collage projects or a build your own Hi-Fi rack but, really, no, just no.
 
Don't tick the check box and put FYI it isn't part of the installation
If it has a big hole in it is a C1, not an FI. And of course it's part of the installation. Just because we're not permitted to work on it doesn't mean we just pass the buck and ignore a potentially dangerous, or imminently dangerous condition.
The regulations state that we should get the person ordering the work to report the condition to the DNO. The DNO will want to know what that condition is.
Is this a windup?
 
The holes in the top where the cables come out may have to much of a gap and may warrant a code 3 . But I would not code it myself unless I'm missing something.
 
You see lots of these service heads with plywood or Bakelite across the top of them and I only normally comment if they are not secured in place, they are often split to fit them. Not sure if in the pic it is actually fixed in place.
 
C2 and a call to DNO. If left as a C3, there's the potential nothing will be done with it, however a C2 would fail the EICR. I'd C2/FI (C2 for the age of the head and FI for DNO I put/action - You could put FI alone which would still fail/unsatisfactory the EICR so swings and roundabouts really.

The whole cop-out of putting the onus on the customer to contact DNO is utter crap as they're in no-way competent to assess the potential hazards and risks presented, you as the competent person however, are; make the call, take a name and reference number and discharge the duty of care.
