Worth looking at the ucl pdf I have posted, basically the switch needs to be labeled, 1. firefighters switch 2. what its switching eg, shop sign 3. opperation pull down to isolate.
So would say its failure would be, operation wrong way round, and no signage
 
surely whipping off the switch front and turning it 180 deg is a 5 min job or am i missing something startlingly obvious ? :)

Could be wired in Pyro....more frightening than a fire, for some.
 
The switch must be painted black and white to hide it between the lines. With out a make / model number, are we sure this is a legitimate fire switch?
Yes I do know what it says in the regs and what the UCL recommendations are.
Is it a genuine fire switch or a bog standard MEM 15A metal clad isolator.
