How to enable a 220V connection from the meter?

Hi everyone!
I am Cristian from Ecuador. Since I understand just the very basics of electric circuits, I would like to ask for some explanation about the wiring on the box containing the meter at home. First of all, I have to mention that meters in Ecuador are supposedly capable to deliver simultaneously both 110V and 220V, however, I am only using 110V by now but I am planning to enable a 220V receptacle. Then, when I was trying to find what I would need to get done for this purpose, I noticed that the electricity company left those 2 pairs of breakers sort of “bridged” with those U-shaped pieces wire and when reading the voltages with the multimeter all 4 breakers supply 110V. How am I supposed to get 220V? Can somebody please help to solve this “mystery”?

Is the second leg even in there ?
As I cannot see it
If you only have 120v ?
Then it's possible that only one leg is present
If so ? then 240 is not possible
As for 240 you need two differrant legs

Normally it's utilising two 110 phases combined, looks a strange set up.

On that 40 amp double breaker on the left measure between both them together on top and see if you have 220vac. If your reading to ground it will be 110vac
