I have no idea. Google picture search augmented by AI (!) just has this to say:

The image shows a vintage Positive Input Ventilation (PIV) control wall plate. These plates are typically found controlling ventilation systems in homes, often installed in the ceiling or high on a wall. The central knob likely controls the fan speed or airflow of the PIV system. Given the appearance, it's probable that the plate is from the late 20th century. Unfortunately, no specific information about this particular model has been found.

You might be able to replicate its function with modern components if you had a good look inside and worked out what it does (maybe just a switch) - if that's what you need to do!
