Not come across this before but the installation is labelled PME system but there is no earth connection supplied by the distributor. The earth connections are stuffed inside a loose connector in the fuse box and a 4mm cable connected off the incoming water supply, obviously not allowed anyway. Has anyone come across this situation where no earth connection is supplied by distributor and why they would not have done it ?

Yes 'PME system' is printed on the DNOs cutout box
The only earth connedction is the 4mm cable which goes from the loose connector inside the CU underground alongside the incoming water pipe so guess this is an earth not bonding connection.
Earth loop test performed but this is only going to the 4mm cable mentioned above
 
So with it looking like there are no conductors coming from the DNO cut-out and therefore the installation is getting it's "earth" from the water pipe, so unless the water pipe comes under reg 542.2.4, then it can not be used as an electrode.

I would now call the DNO and tell them exactly what you have found, a cut out with a PME sticker on it, that has no obvious PEN/PEL connection and remind them that they have a duty to maintain a TNC-S system, if this is what the system is, either way they should send someone out and it may solve the mystery.

Are you going to fit rod(s) to TT the system.

Can you physically see the 4mm connected to the water pipe or could it go to a buried electrode somewhere???

You mention having carried out a loop measurement, what reading did you get and was this with the 4mm conductor disconnected from the 'connector'???
 
Picture would be great

I have a feeling its a modern style PME cutout which get fitted in new build installations, the block is integral with the cutout neutral and you have to knockout a hole on the top and a hole on the front to reveal the allen key termination.
