Test form help

Just wanted to know if a periodic test and inspect form is still valid or if it is now a electrical installation condition report if it is still valid when would I use it compared to the new form


also on the new form section M observations has NO AND YES boxes on the far right. Can someone please let me know what these are for.
Is it to stay if the work has been rectified

The Y/N is to indicate whether further investigation is required for a particular observation.

In most cases, you should be able to allocate Code 1, 2 or 3 to each observation. Therefore, further investigation would not be required for the purposes of the report.

To quote some text from the ESC:-

"Further investigation should not be called for in respect of any observation UNLESS that investigation could reasonably be expected to reveal danger or potential danger...."
