TNCS Service head, earthing and bonding

Went to quote for customer's dis board upgrade today found this gem! looks like 6mm main earthing, main bonding etc going into the MET within service head.. also no plastic case on head. I presume I would contact DNO to resolve and provide 16mm + additional MET? Just haven't come across this yet.
There are other big issues with the installation that are obvious so quoted for EICR previous to DB change. Also don't fully understand why there are two meters still as DNO fitted digital one only a few years ago? I think the other one is redundant as can't see what its supplying... there has never had a rented area within the property to justify another meter..

I had all the Economy 7 gear still present in my own house until earlier this year, even though it hadn't been used for over a decade. Had to take two meter readings each time even though they were both charged at the same rate.

Maybe i'm getting really confused, the earth block is attached to the service head, in the picture it hasn't got a cover like it normally would have... would it be ok for me to put a 16mm earth into that block without the DNO? seeing that its their property?
Is it the same "whose responsibility is it" situation, like it is with the pulling of main fuses, for example? sorry for not being clearer in initial post, been a long day..

and why does the earth block on that cutout need a cover?...

There was never a cover over the earth block on that type of cutout. It is left exposed for the electrician to connect the main earth conductor to so if you are an electrician JFDI.
Modern cut outs have either drilled holes or a removable cover for access to the earth block screws.
