It's not American, it's clearly of European heritage by its design and features. I suspect it is a Swiss type but perhaps 40 years old and prior to their present SEV1011 standard which has some similarities. The flat sides and combination of round and flat phase pins are typically Swiss. Many European countries had / have similar-but-different 10-16A 3-phase plugs that evolved from the same roots as their single-phase types. What is the make / nationality of the machine tool it's attached to?

If you want to replace it, you'll have a job finding one! A more modern European 3P+E socket might fit in the same panel cutout though.

In the meantime I'll see if European plugmeister Oof Oud recognises it...
 
IIRC on a Perilex only the earth pin is flat, the phase and neutral pins are all round. A similar situation applies to the other currently available European plugs that have one flat pin. Here, the flat blade is one of the three phases, which I take as a hint that it might be Swiss. The reversed earth contact gender is also very non-German.
 
Oof has confirmed that it is the old Swiss 'type 5', which was still in use in 1978 but discontinued some time after that. The 'type 15' which is similar but 3P+N+E with round and flat pins is still a current type of plug in Switzerland.

Your probably right, the machine is an Aceira F3, it's Swiss and probably 40 years old, so very impressive!
i appreciate the help and yes, much easier to change to a new style plug and socket!
