The top hole leads to the lamp the bottom to one of those twisty spiral bedside lamps you get in hotel rooms.

Yeah it is sheathed at the top but not the bottom and neither are the brown and blue you can see.

The wires in the flexi tube should be sheathed.

Connector block inside a plastic box is ok.

Its the brown and blue cables tho. One little nick and that fitting could potentially become live.

I'm no expert but having single insulated conductors inside the metal box doesn't seem like it fulfills the requirements.

Exactly.
The metal box just provides mechanical protection.
Everything inside the metal needs to be double insulated.
