I have seen a lot of crimps fail due to improper use of tools and the incorrect amount of conductor showing. When working on london underground trains we had so many fail that we had to strip an exact amount on the cable and use calibrated crimping tools!
 
Overcrimping is caused when you have an adjustable set and deliberately set crimp to a lower lug size assumably the thought on this is to get a tighter crimp, it valleys a circular crimp into a u-shape and can weaken the structure of the crimp itself, yes im a bit of a perfectionist i suppose but im not to NASA's standards as solid cores and tinned strandeds are a big NO! NO! ....

Aye ... well that's why aahh aalways SOLDER mine lyke ... ye cannit beat a propa soldered joint.

These are the major causes of failures. All too often even a good crimping tool in the hands of a novice adds up to a good many crimp failures... lol!!!

Darkwood, your now talking about very specialised installations now, that are far, far outside any of our requirements!! lol!!!

Exactly as i stated above, but that is always down to the failure of the operator not the crimp lug or the crimping tool....
