test results are unique to each installation, look in the back of the onsite guide, I am sure there is an example one filled out.
Just to be picky, by the sounds of it you're talking about 'schedule of test results' sheets, not EIC's, though of course you need one in order to provide the EIC.
are you qualified or are you going nthrough training scheme?
if your qualified and are using the terminoligy your using you need to go back to school
if your training you may need to take a step back as you seem to be getting a bit ahead of your self
third option is your a DIY joe who had got hold of blank cert ( I have a real problem with certs being available to ALL and sundry ) and youwill fill in results from someone elses sheet different circuits ect ect as your own hence NOT acctualy testing anything
Option 4 Call a local qualified NICEIC approved CONTRACTOR and ask him if he will do the part 3 of the D,C.T. part of full EICert
