This is assuming the fan or any other equipment was something you installed as part of the job ?.
I am very happy to have this debated, but as far as I can ascertain the boxes on an EIC are only entered for the work you have done NOT existing equipment. I think this is a common error and one I have fallen in to.
 
if the fan was 12 volt but the transformer was in the fan would this still be under selv.
would you need to tick electrical seperation box and band 1 and 2 circuit seperation box.

Agreed, neither as you have lv rated insulation to the cable supplying the fan, and SELV does not fall under the banner of 'electrical seperation'.

Absolutely, an EIC is only for work you have carried out, if you have not done anything to do with 'electrical seperation' or segregation of band 1 & band 2' etc then it's an N/A in the box.
 
There seems to be some confusion about the boxes to tick in general , the new periodic inspection report , the condition report or whatever its called , has even more boxes to tick - god help us all .
 
so you would tick selv box and leave electrical separation with n/a and band 1 and 2 with n/a.

I don't have any axe to grind with the NIC neither do I have a need to promote them, but I found the instructional DVD's on form filling really helpful, it shows a lot of common errors even experienced sparkies make very useful I found, tho the measured Zs issue I tend to agree with the majority on here and hence the purchase of said Lightmate adapters.
