That old chestnutAlso,check for correct configuration,as a lead with a male type plug at both ends,could pass inspection,in regard of how it is fitted.
Industrial sites are notorious,for "modified" leads,with the 110/240 16A "universal-with-the-nib-removed" being common
As written in electricity at work actWe have them at work , buy the scafftag type label for test dates they tie wrap on and are very hard wearing

Visual check , must have metal braiding if on construction site or engineering shop floor etc (type 7 cable) or sy which we like for portable equipment ( some people don’t like to use )
